North Carolina offers portable disability parking placards and disability license plates tied to an eligible vehicle. The application must identify the qualifying person and the appropriate medical certification category. Pay particular attention to renewal: a five-year disability placard and a total-and-permanent placard have different medical recertification rules even though both expire.

Choose the credential for the person being transported
NCDMV's disability parking instructions describe placards and plates. A placard can move between vehicles transporting its holder, making it useful when family members, taxis or caregivers provide rides in different cars. A disability plate belongs on the vehicle registered under the qualifying arrangement. Select the credential based on the person's transportation needs rather than assuming the person must hold a driver license or personally drive.
The medical criteria appear in G.S. 20-37.5. They include specified limitations in walking, use of assistive devices or portable oxygen, qualifying cardiac or lung conditions, severe walking limitations from certain conditions and qualifying visual impairment. A diagnosis alone does not establish every part of the statutory test. The authorized certifier determines whether the applicant meets an applicable criterion.
Understand the three placard certifications
The July 2026 MVR-37A application, read October 21, 2026, distinguishes three choices. An ordinary disability parking placard lasts five years and generally needs medical recertification for renewal. A temporary placard lasts the provider-selected period of 1-6 months. A total-and-permanent disability placard also lasts five years but does not require another medical certification at renewal when that status is already established in the DMV record.
This distinction matters when reading a page that loosely calls all long-duration placards permanent. The total-and-permanent designation removes a repeat medical requirement; it does not create a credential without an expiration date. Ask the certifier to select the category supported by the condition, and verify that the issued placard record reflects that category before the next renewal.
Have the proper professional complete the medical section
The certifier rules in G.S. 20-37.6 recognize specified licensed medical professionals, the Division of Services for the Blind and a qualifying U.S. Department of Veterans Affairs disability determination. The application explains which information the provider must supply, including the provider's signature, license number and the patient's identity. Complete your portion without changing the medical professional's certification.
A licensed certified nurse midwife may certify an initial temporary-placard application under the statutory exception. That exception should not be extended to every plate or permanent-placard transaction. If a parent signs for a minor or someone acts under a power of attorney, follow the applicant instructions and supply the required authority. A caregiver's practical role does not automatically replace the signature or documentation the form requests.
Prepare the application, payment and delivery method
MVR-37A lists a $5 charge per placard, as verified October 21, 2026. Applicants without a disability plate may request an additional placard within the statutory limit; a qualifying disability plate owner is eligible for the accompanying placard described in the law. Decide how many are needed for the transportation arrangement, and protect each placard and its matching registration card from loss.
Take the completed materials to an office shown in NCDMV's license plate agency directory, or use the mailing address on the application. The July form describes same-day agency service and an estimated 3-4 weeks for mail delivery, read October 21, 2026. Those options assume an acceptable application; missing certification or an unresolved identity issue can prevent issuance during the visit.
Renew the placard using its recorded category
The MVR-37AA renewal application distinguishes whether medical recertification is required. Under the application and statute read October 21, 2026, the usual five-year renewal cycle remains in place, renewal may begin within 90 days of expiration, and medical recertification is not required for a qualifying renewal at age 80 or older. A previously established total-and-permanent certification supplies another exception.
Check NCDMV's disability page for the available online renewal route and the status recorded for your placard. If the credential is temporary and the disability continues, obtain the new application and medical certification required for another temporary period. Keeping an expired placard in the glove compartment does not itself extend the parking privilege while new paperwork is being prepared.
Apply separately for a disability license plate
Use MVR-37 for the disability plate. It requests the vehicle identification and provides a guardian or parent section where applicable. The statute permits the registered owner to qualify based on the owner's disability or the qualifying parent or guardian arrangement. Match the application to the title record rather than applying under the name of whichever relative is available to visit the office.
The plate application carries the regular applicable plate charge, and plates renew annually, as confirmed on the form and statute October 21, 2026. Review the vehicle fee information for the registered vehicle's class. A parent or guardian arrangement also has a five-year recertification requirement under the statute. Keep that obligation separate from the vehicle's annual registration renewal so neither task is overlooked.
Treat personalization as an additional request
An owner seeking custom characters on a disability plate can review MVR-27H. The form read October 21, 2026 lists a $30 annual personalization charge in addition to the regular license fee. It provides a preference list and its own character-space instructions. An ordinary personalized-plate request does not establish medical eligibility for a disability plate.
Consult NCDMV's license plate guidance before changing an existing plate. Retain the current lawful plate until the replacement instructions permit the change, and verify the assigned number in the registration record. Personalized wording does not broaden the parking privileges available to the qualifying person.
Display the placard and keep its card with it
The placard must be visible while the parking privilege is being used, ordinarily hanging from the interior rearview mirror. The law allows the driver's side dashboard placement when there is no suitable inside mirror or the holder cannot reasonably hang it. Its registration card must accompany the placard in the vehicle. Check that the card and placard numbers correspond when more than one household vehicle is involved.
The qualifying person must be the driver or passenger for the parking privilege to apply. Someone may lawfully drive a vehicle bearing a disability plate without that person aboard, but may not use the person's reserved parking rights during that trip. Remove any hanging item that obstructs the driving view before moving the vehicle, and store the credential where the holder can retrieve it for the next ride.
Use accessible spaces without blocking access
The U.S. Department of Justice's accessible parking guidance explains the function of access aisles and van-accessible layouts. The striped space next to an accessible stall is needed for movement and equipment deployment. It is not an extra parking space for a small vehicle, a motorcycle or a caregiver waiting nearby.
North Carolina extends qualifying parking privileges in time-restricted zones, subject to the statute's exclusions for prohibited parking and spaces reserved for special vehicle types. A credential does not authorize blocking a curb ramp, loading zone, fire lane or property entrance. Read the signs at the destination and leave enough usable space for another person's ramp or lift, even when your own passenger does not use that equipment.
Replace the item that was actually lost
The current placard application separates replacement charges. As read October 21, 2026, replacing a lost or stolen placard costs $5, a replacement placard registration card costs $25.50, and a damaged placard is replaced without a charge. Tell the plate agency whether the missing item is the hanging credential, its matching card or both. A request for the wrong document can leave the holder without the evidence required during use.
Maintain the vehicle's other requirements
A disability plate does not remove the registered vehicle from North Carolina's liability insurance rules. The owner must also address any applicable vehicle safety inspection. Disability parking eligibility, medical fitness to drive and the vehicle's registration status concern different facts. Arrange a ride with a properly authorized driver when the holder uses the credential as a passenger.
Frequently Asked Questions
Can a person who never drives receive a placard?
Parking eligibility concerns the qualifying disability and transportation of the holder. A passenger can use an issued portable placard in a vehicle carrying that person, with the matching registration card and proper display.
Does total-and-permanent certification eliminate future renewal?
It removes the repeat medical certification requirement when that designation is established in the record. The placard still has an expiration date and must go through the applicable renewal process.
Can a relative borrow the placard for a personal shopping trip?
Reserved parking privileges apply when the qualifying holder is being transported. A family relationship or permission to borrow the car does not authorize someone to use those privileges without the holder.
What should I do if a temporary disability continues?
Obtain the new application and provider certification required for another temporary placard. The expired temporary credential is not converted into a permanent one merely because recovery takes longer than expected.
Is the striped area beside the stall available for parking?
That area provides access for people using mobility devices and vehicle ramps or lifts. Leave it unobstructed regardless of whether the neighboring stall is occupied at the moment you arrive.
Does an ordinary personalized plate prove disability parking eligibility?
Custom characters and disability certification are separate requirements. Use the disability plate process and, if desired, the specific personalization application instead of treating a standard personalized plate as a parking credential.
